Personal Health Coaching allows you to work privately, one-on-one or as a couple/family, to work on a customized plan personalized to meet your specific needs and concerns. You'll have unlimited access for coaching/education to give you the confidence to successfully implement healthful lifestyle changes.
We provide a collaborative program that includes coaching appointments (in-person or remote) and mobile touchpoints within a framework of mindfulness to focus on creating sustainable health behavior change. Through our licensed use of the Nudge Health Tracking App, you can sync data from any of over 100 leading wearables and health programs/devices.
Personal Health Coaching sessions are available as a stand-alone service or to compliment your LEAN30 Membership. Sessions are scheduled as 30-minute or 45-minute appointments, either in-person or remotely via audio/ webconference & online chat. You may add additional participants to create a duo or family/private group (limited to 6 participants). Personalized sessions may include any or all of the following:
Assessments, motivational interviewing, goal-setting and coaching to incorporate sustainable healthy habits (eating well, exercise, positive mindset, rest, stress management).
Nutrition education and guidance with meal plans, recipes, grocery lists, prep/cooking strategies & supplementation recommendation(s) specific to your needs.
Journaling, worksheets or assignments for benchmarking, re-assessments and setting new goals based on progress.
Mobile Touchpoints - Weekly check-in/follow-up via in-app messaging for personal Q&A, increased accountability to stay on track to meet your goal(s) or successful maintenence - unlimited messaging.
Health Data Monitoring - We keep the logging process simple, increasing the likelihood that you'll use it as it will serve as a great overall snapshot of your holistic health behaviors so we can see trends, spot potential problem areas and get a sense of where to dig deeper with personalized coaching and effective methodologies.
